The aim of the integrated health and wellbeing service is to provide greater positive health impact
on the lives of our residents, particularly targeting those who will most benefit through a service that motivates, empowers, and enables communities to achieve their best health and wellbeing and reduces the widening gap of health inequalities.
The service will predominantly cover the following lifestyle services, physical activity, weight management and cardiovascular disease.
